Each County Board of Developmental Disabilities keeps a list of providers that wish to receive notifications when an individual is looking for a provider. The information about how to be added to the list is available on each county's website. The websites also have information about the process to become a provider, renewal of provider certification, and other resources for providers. Click the county below to see the contact information.

A provider can extend services when the person is not physically able to provide Transportation on their behalf – ex: buying groceries. A driver abstract is a three-year record of all moving violation convictions, accident involvement reports and other actions that result in license suspensions, revocations or other disqualifications. Get this from local DMV.
Note: A person having 6 or more points on their license cannot provide these services.

Service Delivery Documentation
Requirements- For any service that you provide, you must have documentation of that service. Each service has its own documentation which is found in the rule for each service. The forms can appear anyway that you like, but it MUST have all required elements. DODD provided templates: https://dodd.ohio.gov/compliance/Office+of+Compliance/Provider-Resources

Annual Training – Independent providers are required to complete a total of 8 hours of annual training. Completing the first section of this course satisfies the requirement for independent providers to complete two hours of department-provided annual training.
The additional 6 hours of training can be taken from any source. This training needs to be relevant to services provided and people served that relate to components of quality care, health and safety, and/or positive behavioral support
Recertification – Provider certification is valid for three years. After three years, the providers must submit a renewal application to maintain their certification. Can reapply up to 90 days in advance of expiration.

Yes, you are required to have an in-person session of First Aid/CPR. Some Developmental Disability County Board offices offer this to people that live within their county. Otherwise, you will have to reach out to local fire dept or a local Red Cross.
Yes, taxonomies are the services that you can possibly provide. Suggest that you put more then you need, that way you do not have to go back later to update.
Maybe, sometimes it will just take the info entered and other times they will say you have to upload a voided check. To bypass this, just upload a voided check!
As an independent provider this is your Social Security Number.
You will use seven number 9’s – ‘9999999’ to complete the training. You will need to go back into Sandata to update this number once you are certified.
Yes. You need to apply for NPI and have this number to complete the PNM process. **Note NPI can take up to 14 business to receive.
A: Shared Services is WHO will process your payments after service and billing is done. You MUST have a Supplier ID through Shared Services for them to process. Can take up to 14 business days to obtain the Supplier ID number. You will get this number via email after you go through the steps to apply. This email will need to be included when you apply for your DODD Waiver Certification application.
EVV is the ‘clock in and clock out’ system. This is required under the 21st Century Cures Act. A federal law was passed in December 2016 that states must implement an electric system to verify certain home and community-based services were delivered in order to continue receiving federal financial support toward the costs of those services.
National Provider Identifier and Taxonomy Guidance for Providers of DODD Wavier Services. In accordance with Administrative Code rule 5160-1-17, all providers with an available taxonomy are required to have a National Provider Identifier (NPI) on file with ODM.
PNM offers a centralized portal to reduce administrative burden on providers by streamlining the process for provider certification. This routes you from PNM (Medicaid) to DODD (PSM) application portal.
Yes. If you try to upload the documents together you will get an error message. Click on the document you want to upload- CPR for example. Then choose document- once you choose that then click upload document. Again, you need to do this with every document in the application.
